These are the steps…

First you’re going to want to open a new document in Photoshop. I made mine 585×185 pixels

Next write out your text. Make it whatever font you’d like. I’m using BEBAS because it is a thicker font. Make your text color #000000. Feel free to kern your text at this point if you’d like. You should have something similar to this…

Now you’re going to want to right click on your text layer in your layer palette. Select Blending Options and choose Gradient Overlay. Apply these settings with whatever gradient you want.

Your text should now look similar to this…

Now create a new layer in your layers palette. Drag the new layer below your text layer. Click on your text layer and press CTRL+E on your keyboard. This should merge both layers together. Your text will no longer be an editable text layer.

Next right click on your new layer and select Blending Options and match these settings…

Once again make a new layer and drag it under the text layer. Click on the text layer and press CTRL+E to merge the layers together.

Right click on this layer and select Duplicate Layer. You should have 2 identical layers now. Choose the bottom layer and click on your Move Tool in your toolbar. Press the down arrow on your keyboard 3 times and then the right arrow key 3 times. Your bottom layer should be slightly below and to the right of the original text.

Now go to Filter > Noise > Add Noise and match my settings.
